State-Based Diabetes Prevention & Control Programs
Vermont
The
Vermont Diabetes Prevention and Control Program (DPCP) began receiving
funding from the Centers for Disease Control and Prevention in October
1996. Activities supported by the Vermont DPCP include the following:
- Surveillance. Diabetes-related data will be collected from
several sources, including the Center for Public Health Statistics;
the Vermont Department of Banking, Insurance, Securities and Health
Care Administration; Medicaid; Medicare; Blue Cross and Blue Shield
of Vermont; and the Community Health Plan, the states largest
health maintenance organization.
- Creation of a focal point for diabetes control within the state
health department. To ensure a coordinated approach to diabetes
control, the Vermont DPCP will bring state health department representatives
together in a coalition that will develop a statewide plan for diabetes
control activities. The coalition will disseminate and promote existing
standards in diabetes care.
