State-Based Diabetes Prevention & Control Programs

Image of the state of Utah The Utah Diabetes Prevention and Control Program (DPCP) has received funding from the Centers for Disease Control and Prevention since 1980. Activities supported by the Utah DPCP include the following:

Collect, analyze, and distribute data on diabetes. The Utah DPCP has developed a state surveillance system for diabetes. Data sources include death certificates, primary care practitioner surveys, consumer surveys, the Behavioral Risk Factor Surveillance System (BRFSS), the Utah Health Status Survey, birth certificates, and hospital discharge records. A diabetes surveillance report is distributed every 2 years. Topic specific reports (e.g., gestational diabetes, eye exam) are produced annually.

Develop and produce culturally and linguistically appropriate patient education materials.  The Utah DPCP, in collaboration with the Diabetes Treatment Program at St. Mark’s Hospital, has produced and distributed a patient education booklet in Samoan, Korean, Serbo-Croatian, Spanish, low level English and Tongan. The booklet is also being translated into Russian, Vietnamese and Farsi. 

Develop and support community-based diabetes activities. The Utah DPCP coordinates through local health departments, community health centers and community-based agencies representing underserved groups to provide awareness activities at the local level. The Utah DPCP provides training, technical assistance, supplies and educational materials to the local entities.

Develop and support projects to improve clinical quality in health care delivery systems. The Utah DPCP has a voluntary state-sponsored certification process for outpatient diabetes self-management training programs to improve the quality of diabetes education throughout the state. The certification process is based on national standards and incorporates diabetes clinical quality improvement. Programs that receive certification will be required to collect data on outcomes and use the data to improve their processes. The Utah DPCP provides training and technical assistance to diabetes education program staff on request.

Assure health insurance reimbursement for diabetes self-management, supplies, and equipment. The State of Utah has a law requiring managed care organizations (MCO) and health maintenance organizations (HMO) to pay for supplies, equipment and self-management training for people with diabetes. This was re-authorized during the 2004 legislative session. A study by the Insurance Commissioner found no increase in costs to the Health Plans during the initial 3 years of the mandate.

Create public awareness campaigns on risk factors for diabetes and prevention methods. Radio, newspaper, bus board and television campaigns have been geared toward Polynesian and Hispanic/Latinos as well as the general population.

Sponsor professional education activities. The Utah DPCP held conferences on diabetes for local public health staff, made presentations at conferences, and collaborated with the Utah State Extension to provide in-service training. A monthly tele-medicine education series is accessed by several sites in and out of state and is viewed by over 20 professionals per month. The DPCP also works with the community health centers in the breakthrough collaborative to provide updates on diabetes. The Utah DPCP developed a training guide to be used by diabetes educators in the community health centers and American Indian programs entitled Diabetes Education Manual. The DPCP also produced Diabetes Practice Recommendations and distributed to physicians throughout the state by mail and in person. 

State strategic plan. The Diabetes in Utah Alliance reviewed the strategic plan within the past year and recommitted to its goals and objectives. The Utah DPCP has also taken the lead in coordinating diabetes-related efforts in the public health system and recently completed an assessment of the Statewide Diabetes Health Care System. Priorities for improvement to the system are being developed and a program improvement plan will be written during fiscal year 2005.
