State-Based Diabetes Prevention & Control ProgramsSouth Dakota
Surveillance. South Dakota DPCP personnel monitor multiple data sources including the Behavioral Risk Factor Surveillance System (BRFSS), Department of Health-Recognized Diabetes Education Program data and hospital discharge data to track the burden of diabetes within the state, measure progress toward state and national objectives, and guide program development. Diabetes Care Management System (DCMS). DCMS is a diabetes registry with care management, education documentation and outcomes tracking capabilities provided free of charge to health care professionals. The system assists health care professionals to use preventive care practices that follow national diabetes care guidelines. Diabetes Education Recognition Program. The DPCP coordinates a quality improvement and assurance program for health care facilities wishing to provide diabetes education in accordance with national standards and receive reimbursement from insurance companies and Medicaid. This program provides technical assistance for facilities that wish to receive national recognition. Guidelines for Diabetes Care. Clinical guidelines for the care of people with Type 1, Type 2 and Gestational Diabetes Mellitus were published in January 2004. These guidelines were sent to more than 2000 primary care providers in SD and posted on the www.sddiabetes.net* site. Information for People with Diabetes. The SD DPCP promotes awareness of self-care techniques and regular provider visits through the Link program. South Dakotans with diabetes register and receive the Take Charge of Your Diabetes book developed by the CDC, a cookbook and a variety of brochures developed by the DPCP on topics such as diabetes and your heart, diabetes and your mouth, etc. Quarterly, these people receive further information on topics such as influenza and pneumoccoccal vaccinations, nutrition, exercise, etc. Technical assistance. The South Dakota DPCP provides technical assistance to other state agencies on issues specific to people with diabetes such as (1) the Department of Education on issues of children with diabetes in schools and (2) the Department of Social Services on issues related to provision of care and education through Medicaid. Professional education. A statewide multitrack professional training conference is offered annually. This 2-day conference prepared health care professionals to provide comprehensive diabetes care and education in their communities. Contact informationDiabetes
