State-Based Diabetes Prevention & Control Programs

The South Carolina Diabetes Prevention and Control Program (SC DPCP) has received funding from the Centers for Disease Control and Prevention since 1977. Activities supported by the SC DPCP include the following:

  • Health Systems technical assistance. SC DPCP Health Systems technical assistance focuses mainly on the community health centers that are involved in the Diabetes Collaborative. These centers use the Chronic Disease Model and a quality improvement model to improve their health system, delivery system, decision support, information systems, and community resources to help diabetes patients better self-manage their disease. The DPCP also provides technical assistance to facilitate improving diabetes care in office-based practices in medically undeserved areas of the state, and to increase diabetes self-management skills in patients who attend these primary care centers for diabetes care.
  • Surveillance. The SC DPCP monitors the burden of diabetes using data from the Behavioral Risk Factor Surveillance System and other diabetes-related data. The data are analyzed and reported in The Burden of Diabetes in South Carolina, which describes the magnitude of the diabetes problem in South Carolina and the availability of relevant programs and services.
  • Local Diabetes Coalitions. There are 27 coalition chapters across South Carolina, and members include community people, health professionals, and people living with diabetes. The coalitions provide a forum for local communities to plan and implement diabetes-related activities that are locally driven and controlled, to share resources, to create diabetes awareness, to improve communication, to collaborate with coalition members from other communities and to solicit corporate support for community projects. SC DPCP annually provides mini-grants to local coalitions to help them build and sustain the necessary diabetes support infrastructure. The grants also help grantees develop and implement a plan to address diabetes related issues in their community. The mini-grants facilitate the coalitions in developing an internal structure and developing/implementing a plan to address the issues surrounding diabetes in their individual communities.
  • Diabetes Today Training. The SC DPCP provides CDC's Diabetes Today training across the state for communities that are interested in forming coalitions to reduce the burden of diabetes in their community and in developing interventions to promote improved diabetes prevention and control.
  • Diabetes 101. The SC DPCP provides "Diabetes 101" sessions to entities across the state that request updated information on preventing and managing diabetes. These sessions are designed to promote awareness of diabetes, its signs and symptoms, its risk factors, and opportunities for prevention to churches and other community groups.
  • Annual African American Conference on Diabetes. Held every November in Columbia, SC, in observance of National Diabetes Awareness Month, the conference targets people living with diabetes, their caretakers, healthcare professionals, and other interested community members. Participants gather information on innovative programs in diabetes education and in self-management techniques for controlling the disease. There is a minimal ($5) charge for the conference, and registration is required. The first conference was held in 1996 and drew 183 participants, and by 2003 registration had grown to 1010. Each year, concurrent sessions are held on foot and eye care, nutrition, physical activity, depression, medication and monitoring, ask the doctor, and other subjects. Past cosponsors of the conference have been the SC Cardiovascular Health Division and the SC Division of Tobacco Prevention and Control.
