State-Based Diabetes Prevention & Control ProgramsNew Jersey
Surveillance. The New Jersey DPCP uses multiple surveillance
sources to define the diabetes burden in the state. By September 2004,
the New Jersey DPCP will update and finalize The Burden of Diabetes
In New Jersey: A Surveillance Report, a compendium of statistical
data and interpretive narrative addressing diabetes in the state. The
report includes data on demographics, diabetes prevalence, gestational
diabetes, diabetes and health care utilization, and diabetes related morbidity
and mortality. The updated report will also include data to define diabetes
prevention-related issues. Diabetes Quality of Managed Care. The New Jersey Department
of Health and Senior Services annually publishes an HMO Performance
Report.
This report card compares how well health care plans deliver
important health care services and how members rate the services they
receive. Included are two performance indicators for diabetes care (i.e.,
blood sugar testing and eye examinations for people with diabetes). Health Systems Interventions. The New Jersey DPCP produces
a newsletter, Health Alert in Diabetes Care, which is distributed to primary
care providers statewide. Each publication highlights a critical current
issue in diabetes care. The DPCP provides technical assistance and support to the health centers participating in the Bureau of Primary Care Diabetes Health Disparities Collaborative. Community-Based Interventions. The DPCP provides funding
for Southern Jersey Diabetes Outreach and Education System (DOES) program.
DOES targets five counties in southern New Jersey; Atlantic, Cape May,
Cumberland, Ocean and Salem. The program works with individuals and organizations
to increase public awareness about diabetes and its complications. DOES
is also implementing a project to educate providers on prevention issues
and to promote the National Diabetes Education Program’s "Small
Steps, Big Rewards" campaign. Southern Jersey Family Medical Centers,
Inc., the grantee for the DOES project, is a participant in the Bureau
of Primary Health Care’s Health Disparities Diabetes Collaborative.
