State-Based Diabetes Prevention & Control Programs
New Hampshire
The New Hampshire Diabetes Education Program (NHDEP)has received funding
from the Centers for Disease Control and Prevention since 1994. Activities
supported by the NHDEP include the following:
New Hampshire Diabetes Coalition
As a result of a statewide assessment of needs and services related to
diabetes care and prevention, this Coalition was formed to guide the work of
over 100 diabetes partners throughout the state. Key committees oversee
progress toward the State Diabetes Plan objectives related to Access to
Care, Public Education, and Workforce Development. These working committees
have over 35 active members with diverse backgrounds. The Coalition’s
mission is to improve the health and quality of life for New Hampshire
citizens who have been diagnosed with, or are at risk for, diabetes.
New Hampshire Diabetes Education Program Clinical Site Network –
Diabetes Care Quality Improvement
The NHDEP maintains a network of primary care office practices committed
to tracking diabetes care indicators and to continuous quality improvement
initiatives. Each site designates a “champion” for diabetes-related
services, works to identify and intervene for patients at risk for diabetes,
and monitors diabetes care for its patient population. Its goal is to
increase the percentage of people with diabetes who receive recommended
care. During the past 5 years, NHDEP clinical sites have significantly
increased the percentages of people receiving A1C testing, foot exams,
immunizations, and eye exams across all sites.
Professional Education
The NHDEP supports continuing education for health professionals caring for
people with diabetes, in partnership with the state’s Area Health Education
Centers. Its goal is to increase the percentage of NH health care providers
who adhere to the current guidelines for diabetes care. Each year,
attendance at professional education sessions has exceeded objectives, and
evaluations of program content have been almost uniformly positive.
Offerings include:
Annual Statewide Conference: At this 2-day conference, nationally known
speakers present on current topics in diabetes care to an audience of over
250 primary care health providers, insurers, legislators, podiatrists,
school nurses, occupational health nurses, and other health and human
service professionals.
NH Guidelines for Diabetes Care
The NHDEP first published clinical
guidelines for diabetes care in the primary care setting in January 1998.
These guidelines are endorsed by the major health insurers in NH and are
updated each year to conform with current ADA Guidelines. Copies of the
guideline packet are available free to all NH primary care practitioners.
Lunch and Learn Sessions and Inservice Training: Through the NH Area Health
Education Centers, the NHDEP supports on-site professional education for
health care providers at their offices or other local venues. These sessions
address current diabetes management and offer the opportunity for in-depth
discussion of challenging cases and technical assistance in the use of
diabetes monitoring equipment.
Conference Speakers and Presentations
NHDEP provides speakers, poster
sessions, and presentations at professional meetings sponsored by many other
professional organizations, including the NH School Nurses Association,
statewide Nurses’ Association, NH Optometric Association, Medical Office
Management Association, Primex Insurance, Minority Health Coalition, North
Country Health Consortium, American Diabetes Association, and many others.
Public Education
The NHDEP offers diabetes education materials to the general public,
including information on diabetes risk assessment, influenza and
pneumococcal immunization, hemoglobin A1C patient information brochures,
patient wallet cards, blood glucose logs, guidance on foot care, dental
care, smoking cessation, healthy eating and physical activity.
NHDEP works with its partners to distribute these materials at a number of
public events and through statewide media. Each year, the number of
materials distributed has increased, as has the reach of public service
announcements.
Surveillance
Each year the NHDEP compiles a summary of data related to diabetes risk,
prevalence, complications, and mortality, using information from the
Behavioral Risk Factor Surveillance Survey, hospital discharge and renal
data sets, vital statistics records, and other relevant sources. In recent
years the NH Diabetes Data Book has included increased information on
diabetes risk factors in the population, as well as diabetes-related data
specific to ethnicity and local geographic area. A summary of clinical
measures tracking the quality of diabetes care in the NHDEP primary care
site network has also been added to this annual surveillance report.

New Hampshire Guidelines for Diabetes Care
This packet summarizes recommendations for the current standard of diabetes
management according to ADA Clinical Guidelines. It includes provider
references, chart flow sheets and reproducible patient materials.
New Hampshire Diabetes Education Program Description
This packet provides basic information about diabetes for the lay
audience. It also includes descriptions of the organization, objectives, and
activities of the NH Diabetes Education Program.
New Hampshire Diabetes Data
This publication summarizes annual data as noted above under NHDEP
Surveillance activities.
New Hampshire Action Plan for Diabetes
This publication summarizes the statewide diabetes needs assessment process
and results, and describes the development of goals, objectives and
priorities to improve the health and quality of life for New Hampshire
residents affected by diabetes. The Plan will guide the work of New
Hampshire’s Diabetes Coalition partners during 2005-2010.
