State-Based Diabetes Prevention & Control ProgramsNebraska
Publication of surveillance data. In 2003, the Nebraska DPCP released a revised version of its report The Burden of Diabetes in Nebraska, which the program last published in 1997. This report presents state-level and, in some cases, county-level data from birth certificates, death certificates, hospital discharges, the Behavioral Risk Factor Surveillance System (BRFSS), the Youth Risk Behavior Survey (YRBS), and the U.S. Renal Disease System. Promoting quality diabetes care within health care delivery systems. The Nebraska Diabetes Consensus Guidelines were first developed in 1997 by a DPCP-convened task force, that consisted of health care providers, insurance company representatives, and representatives of professional and other organizations with an interest in diabetes. The guidelines were designed to inform clinicians of the type and frequency of medical tests and services that are essential to ensure the highest quality of care for their patients with diabetes. The DPCP has convenes the task force annually to update guidelines, and distribute them to health care providers throughout the state. The Nebraska DPCP also works with interested primary care providers to initiate quality improvement activities within their clinical setting, and with the three community health centers that are participating in CDC’s Diabetes Collaborative. Recently, the Nebraska DPCP has partnered with the Nebraska Lions Club for their “Focus on Health” initiative, that offers diabetes-specific, on-site training to health care professionals, and quality improvement and community awareness components. Encouraging communities to implement their own diabetes prevention and control activities. The Nebraska DPCP uses the CDC Diabetes Today model to stimulate the development and implementation of diabetes prevention and control activities at the local level. This model incorporates community assessment, coalition development, design of intervention and awareness strategies, and evaluation. The DPCP is currently providing support to three Nebraska communities that have diabetes coalitions. Increasing public awareness. The Nebraska DPCP uses materials developed by the National Diabetes Education Program (NDEP) to promote public awareness of diabetes-related issues. NDEP campaigns conducted in Nebraska include “Be Smart About Your Heart: Control the ABC’s of Diabetes,” “ Small Steps, Big Rewards,” “Control Your Diabetes for Life,” and flu awareness. During 2004, the DPCP will implement, in collaboration with the Nebraska Health and Human Services System’s Division on Aging, a foot care initiative based on the NDEP’s “Feet Can Last a Lifetime” campaign. The DPCP’s Diabetes Newsbeat newsletter, with a statewide circulation of approximately 9,000, also promotes awareness of key issues among people with diabetes throughout Nebraska. Providing professional education. The Nebraska DPCP provides and co-sponsors in-service training programs, workshops, and symposiums for health care professionals throughout the state. The DPCP also maintains an extensive storehouse of diabetes-related patient and professional education materials. These materials are free to health care professionals who request them.
