State-Based Diabetes Prevention & Control ProgramsMississippi
Professional Education. In partnership with the state's peer review organization and the Diabetes Foundation of Mississippi, the DPCP conducts 2 annual diabetes education workshops for healthcare professionals. The workshops are designed to provide the knowledge and tools to increase the use of clinical recommendations for people with diabetes. Each workshop provides up to 8 hours of continuing education credit. Lower Extremity Amputation Prevention. The DPCP sponsors training to improve early recognition and management of independent risk factors for ulcers and amputations and to prevent or delay the onset of adverse outcomes. Trainings are conducted by a Certified Foot Care Specialist and are attended by nurses, educators, and physical therapists. Participants received 8.6 hours of continuing education credit. Community Outreach. Community and faith-based organizations partner with DPCP to conduct diabetes related activities on the local level. Partners receive funding to conduct diabetes education classes, cooking classes, diabetes awareness activities, disease self-management workshops, and diabetes support groups. Diabetes Collaborative. The DPCP partners with community health centers participating in the Diabetes Collaborative. Activities include quality assurance initiatives, diabetes education, and application of clinical recommendations for persons with diabetes. Surveillance. DPCP personnel use multiple data sources to track the burden of diabetes within the state, measure progress toward state and national objectives, and guide program development.
