State-Based Diabetes Prevention & Control Programs

Image of the state of DelawareDelaware's Diabetes Prevention and Control Program is designed to improve access to affordable, high quality diabetes care and service. Priority is given to people with the highest risk for developing diabetes or complications of the disease. The program is funded by a cooperative agreement with the Centers for Disease Control and Prevention (CDC).

  • Collects and publicizes current, accurate information about diabetes.

  • Develops approaches for reducing the impact of diabetes.

  • Promotes lifestyle habits which can help prevent and control diabetes.

  • Implements measures to ensure widespread application of such approaches.

  • Coordinates the diabetes-related efforts of public and private health organizations.
