State-Based Diabetes Prevention & Control Programs
Connecticut
The
Connecticut Diabetes Prevention and Control Program (CT DPCP) has received
funding from the Centers for Disease Control and Prevention since 1994.
The priorities for the CT DPCP are as follows:

Defining the burden of diabetes in Connecticut: Existing data
sources such as vital records and the Behavioral Risk Factor Surveillance
System are used to maintain a diabetes surveillance system. This system
allows the CT DPCP to determine the prevalence of diabetes in the
state, identify population groups that suffer disproportionately from
diabetes, and assess the extent of risk factors for developing diabetes
and its complications. The CT DPCP regularly develops reports, such
as the "Diabetes Fact Sheet" as well as responds to special requests
for analysis.

Improving the quality of care for people with diabetes: Quality
improvement projects targeting managed care organizations and individual
providers incorporate training in integrating current standards of
care, improvement in clinical information systems, and promotion of
patient self-management skills. Health care providers at community
health centers are reached through quality improvement projects developed
in collaboration with the Bureau of Primary Health Care, the Institute
for Health Care Improvement, and the Connecticut Primary Care Association.

Community-based Education Projects: These projects focus on
reaching high-risk populations to increase awareness of diabetes.
The CT DPCP partners with faith groups, local health departments,
and other community-based organizations to effectively reach these
populations. National Diabetes Education Program materials are used
in the print, radio, and TV mediums to increase awareness of diabetes.
