State-Based Diabetes Prevention & Control Programs
Colorado
The
Colorado Diabetes Prevention and Control Program (DPCP) has received funding
from the Centers for Disease Control and Prevention (CDC) since 1977.
Activities supported by the Colorado DPCP include the following:

Define statewide burden of diabetes. In carrying
this activity, the Colorado DPCP collects, analyzes, and distributes
Behavioral Risk Factor Surveillance Survey data, diabetes mortality
reports, and gestational diabetes pregnancy outcome reports; conducts
telephone surveys of diabetes patients, and reviews medical records
of adults with diabetes who are receiving care from local managed
care organizations.

Organize focus groups and disseminate findings.
Through this activity, the Colorado DPCP facilitates development of
new and effective approaches and policies for diabetes control patient
education.

Maintain "Buddy System." This is a network
of health professionals that provide diabetes education in hospitals,
clinics, and public health agencies. They receive bi-monthly articles
and educational materials. Rural educators are matched with certified
diabetes educators for one-to-one consultations.

Publish quarterly newsletter. The newsletter is
for health care providers and coordinating professional education
programs through the Diabetes Prevention and Control Program Advisory Council.
