State-Based Diabetes Prevention & Control Programs

Image of the state of CaliforniaThe California Diabetes Prevention and Control Program (DPCP) has received funding from the Centers for Disease Control and Prevention (CDC) since 1981. Activities supported by the California DPCP include the following:

Surveillance. The Behavioral Risk Factor Surveillance (BRFSS) is the cornerstone of the California DPCP’s comprehensive diabetes surveillance system. These data, along with data on hospital discharges and vital statistics, have been used to develop and distribute county-specific diabetes fact sheets. This is in collaboration with the American Diabetes Association and the California Department of Health and Human Services. The California DPCP publishes an annual BRFSS report, and another report every 2 years, that combines data from this survey with data on hospital discharges, morbidity, and mortality for people with diabetes.

Oversight for the Medi-Cal project. The California Department of Health Services is implementing a project with the University of California-San Diego, the Santa Barbara Health Authority, and Harbor General Hospital-UCLA. This project will develop innovative models of diabetes management from managed care health systems or large fee-for-service Medi-Cal providers. The model, which will involve health departments and local community organizations, will be developed by adapting aspects of the Diabetes Complications and Control Trial model to the managed care setting.

Diabetes Coalition of California. The California DPCP works extensively with the Diabetes Coalition of California (formerly the advisory group to the DPCP), which has representatives from the general public, local health departments, universities, volunteer organizations, pharmaceutical companies, and community-based organizations. The coalition has developed evidence-based guidelines, and is now working to implement them. The coalition has developed a patient survey and a model patient record.

Information sharing with health care professionals. The California DPCP has created a series of electronic seminars on diabetes-related issues for diabetes control and prevention programs around the country and coalition members throughout California. The seminars have proved an effective vehicle for sharing information to prevent and treat diabetes, including information on statewide systems and local community approaches.

Hearing officer training. The California DPCP is working with the training division of the Department of Motor Vehicles to train hearing officers in evaluating people with diabetes.
