State-Based Diabetes Prevention & Control Programs

Image of the state of ArizonaThe Arizona Diabetes Prevention and Control Program (DPCP) has received funding from the Centers for Disease Control and Prevention since 1994. Activities supported by the Arizona DPCP include the following:

Arizona Diabetes Council. The 32-member council has three standing committees: education/clinical practice, surveillance, and advocacy. All are working committees whose active members have diverse professional backgrounds.

Current Projects. The Arizona DPCP works collaboratively with communities, municipalities, and regions to address the many facets of diabetes including risk factors, disease onset, complications, and mortality. The support provided by the program enables the various communities to tailor programs to meet the specific needs of their population. Creating a comprehensive statewide plan includes bringing all the support systems together to ensure the good health and well being of those living with diabetes as well as those who are at risk for the disease.  These support systems must include the medical, social, and work communities. Promoting awareness of the burden of diabetes and making effective change on the entire State Diabetes Health Systems will impact those living with diabetes and is necessary to improve quality of life for everyone.

Health Communications, Health Systems, Community Initiatives

  • Provide surveillance documentation related to the national indicators for diabetes through data collection, assimilation, and dissemination efforts to partners in Arizona
  • Assess the strengths, weakness and gaps in the State Diabetes Health System
  • Support community grant opportunities to develop coalitions of stakeholders to assess community needs specific to the populations of targeted high-risk communities
  • Collaborate with internal and external partners in addressing program specific activities for the Hispanic/Latino and Native American population
  • Develop and maintain a working partnership with participating clinics in the Arizona Diabetes Collaborative to establish a consistent method of health care delivery and accountability for clinics in Arizona
  • Provide opportunities for diabetes education and training for professionals to promote best-practices and standards of care for those responsible for providing medical care for people living with diabetes
