  • Developing an Evaluation Plan
  • Fundamentals of Evaluating Partnerships

Without duplicating previously published information, this guide provides an overview of some fundamental considerations for heart disease and stroke prevention programs and partners as they incorporate DHDSP outcome indicators into evaluation planning and implementation. Although this guide focuses primarily on the use of outcome indicators for evaluation of policy, systems, and environmental change approaches to heart disease and stroke prevention, it also notes how indicators may be used in program planning.
