Community Transformation Grants (CTG) States and Communities

Approximately $103 million was awarded to a total of 61 states and communities throughout the US to serve approximately 120 million Americans. Awards are distributed among state and local government agencies, tribes and territories, and state and local non-profit organizations within 36 states, including seven tribes and one territory.

  • 26 states and communities will work to build capacity to implement changes by laying a solid foundation for community prevention efforts to ensure long-term success. Funding amounts range from $147,000 to $500,000 depending on population size and scope of project.
  • 35 states and communities will implement evidence- and practice-based programs designed to improve health and wellness. Funding amounts range from $500,000 to $10 million depending on population size and scope of project.

All 61 grantees will address the following priority areas: 1) tobacco-free living; 2) active living and healthy eating; and 3) evidence-based quality clinical and other preventive services, specifically prevention and control of high blood pressure and high cholesterol.

Implementation grantees may also choose to address additional areas of disease prevention and health promotion, including social and emotional wellness (such as facilitating early identification of mental health needs and access to quality services, especially for people with chronic conditions), and healthy and safe physical environments.

Awardees include communities in many areas of the U.S. with a disproportionate burden of various health outcomes, including:

  • 15 states and communities in the southern United States, an area with the highest rates of obesity and cardiovascular disease.
  • 29 of the nation's large counties and urban areas.
  • 7 tribes and 1 territory (the Republic of Palau).

In addition, 20% of funds are supporting work in rural and frontier populations, which have disproportionate burdens of chronic disease.
