Registration

We are pleased to announce that the 2007 National Prevention and Health Promotion Summit has received an overwhelming response from public health professionals throughout the country. Due to the high volume of registered attendees, summit registration is now closed. There will not be on-site registration at the summit.

Conference Fees and Deadlines

Full Registration (including payment on or before October 27, 2007) $395
Late Registration (after October 27, 2007) $450
Full Time Student Registration (valid student ID required at check-in) $175
Regular One-Day Registration $250
Full Time One-day Student Registration (valid student ID required at check-in) $135

Note: The Pre-Registration Deadline is October 27, 2007. If your registration information is received after this date, you will not be included in the participant directory. In addition, you will have to go to the on-site registration area to check in at the conference. Please register before this date if you wish to have your registration packet ready when you check in.

Continuing education credit will be given only to those who attend sessions and complete required documentation. Participants can receive continuing education credits by completing the evaluation booklet included with the conference materials. There is no additional charge for continuing education credit.

Payment Method

Online registration is preferred. Online payment can be made by check, purchase order,
VISA, MasterCard, American Express and Discover. If you register online and are paying by check, include the registration confirmation number on the check when mailing in payment. Registration also may be submitted by mail but will not be processed until payment has been received. Make checks payable to Directors of Health Promotion & Education (DHPE). Please send check or purchase order payments to Directors of Health Promotion & Education (DHPE),

Substitutions and Cancellations

Attendee substitutions may be made by e-mail request without penalty up to October 27, 2007. After that date, substitutions will be allowed but a fee will be charged for cancellations. Please submit your substitution and cancellation requests to Valerie Collins at vcollinshrg@bellsouth.net. Cancellation fees are $35 for student registrations, and $75 for all other registrations.

For Federal Travelers Outside the Washington, D.C., Area: Thank you for your interest in the summit. In an effort to be good stewards of public funds, care has been taken to balance attendance. CDC has submitted a list of proposed travelers to HHS. To this end, registration has closed to federal employees who are not already listed on the current HHS travel memo. If you would like to be included on the wait-list, please send your name and justification for travel to OChamberlain@cdc.gov. Following the summit, presentations will be available through the CDC Web site.
