When to submit the CE proposal
The CE proposal should be submitted by the dates in the timeline sent to you. Usually, the timeline calls for a draft Education Planning Tool to be submitted first, followed by a draft CE proposal that includes all documents and requested materials. The CE Consultant will review your draft proposal and let you know if it is acceptable to go before the review committee. For CME, the timing is determined by the regularly scheduled CME Physician Review. Your proposal must be submitted a minimum of three weeks before this meeting.
If deadlines are not met, CEALS has the option of not accrediting your activity.
Your final CE proposal should include all documents and requested supplemental materials. No double-sided copies will be accepted. Submit two hardcopies by:
FedEx/UPS:

or by USPS:

OR you may hand deliver the copies to:

Remember to keep a copy of your proposal for your records.
Please send your consultant a Word version of your electronic files. These will be stored by the consultant in case they need to be accessed in the future.
