Working with Other Organizations
There are specific requirements and terminology you must use if you are working with another organization(s) on your educational activity.
Sponsors - Any organization involved in completing the CE proposal, developing the educational activity, delivering or implementing the educational activity, or completing any post-implementation tasks is referred to as a sponsor.
Written Agreement for Outside Funding and Contributions (WAOFC) - A WAOFC (DOC -360 KB) form is required between the CDC organization affiliated with the educational activity and any commercial supporter. This letter contains the terms, conditions, and purposes for providing commercial support. The WAOFC cannot be modified in any way and must be signed and dated by representatives of both the commercial supporter and the CDC organization. A WAOFC document is provided in the Documents for CE Proposal section - it is the only document that can be used for this purpose.
Any outside foundation involved in handling funds, including the CDC Foundation, must sign the appropriate line on the last page of the WAOFC.
A WAOFC is not required between a CDC organization and another federal agency.
Budget
For each educational activity, you must submit a budget. A Budget Document is available in the downloadable Proposal Packet in the Documents for CE Proposal section.
