Instruction for coding 1994 Diabetes Module Q. 7 (About how many times in the last year has a doctor, nurse, or other health professional checked you for glycosylated hemoglobin or hemoglobin "A one C"?) and Q. 8 (About how many times in the last year has a health professional checked your feet for any sores or irritations?):

If respondent answers one or more for "Number of times," code 1—6; 6 = 6 or more.
