Level 4 Intensive Case Management (by specialists and the YES WE CAN
asthma team)
- Child with poor asthma control, family in need of self-management
skills, and
- Significant complications that require specialized help (social worker,
public health nurse, etc.) from outside the basic team
Level 3 Moderate Case Management (by the YES WE CAN asthma team)
- Child with poor asthma control, family in need of self-management
skills, and
- Significant complication(s)
Level 2 Basic Case Management (by the YES WE CAN asthma team)
- Child with poor asthma control because family is in need of
self-management skills, or
- Family has self-management skills but needs better medical management,
access to medications and /or modest social/psychological support
Level 1 Self-Management Support (by Primary Care)
- Child with relatively well-controlled asthma
- Family has self-management skills
- Relatively stable social/psychological status
