Prevention Is Care™ Resource Kit One

The Prevention IS Care provider resource kit #1 offers a range of tools and resources for HIV primary care providers including patient risk reduction intervention tools, a copy of the 2003 MMWR on prevention with positives, PwP journal article, posters and more. The patient education materials can be used during routine office visits and are intended for patients living with HIV. Patient materials can initiate dialog between the provider and patient and reinforce messages from the physician. All patient materials are offered in English and Spanish.

Provider Resources
deskside resource toolkit image This deskside resource toolkit was created to house the suite of PIC tools and resources for providers working in HIV prevention. The kit is presented in one package designed to reinforce campaign messages that can serve as a resourceto primary care providers who work with persons living with HIV.
 
Behavioral Screeners View Resources >>
Behavioral screener thumbnail image This one-page screener, available in both English and Spanish, contains patient self-administered risk behavior questions and suggested provider discussion topics to help patients reduce risky behaviors.
 
Relative Risk Charts View Resources >>
Relative Risk Charts thumbnail image These charts, available in both English and Spanish, provide patients with information on the relative risk of HIV transmission by type of sex act and with or without the use of condoms and are intended for provider/patient discussions during an office visit.
 
Viral Load Charts View Resources >>
Viral Load Chart thumbnail image These charts shows the estimated number of new HIV infections per 100 person-years among heterosexual serodiscordant couples by serum viral load of HIV partner and are intended for provider/patient discussions during an office visit.
 
Poster thumbnail image Provide patients with information about the impact of their behaviors and prompt them to engage their providers in further discussions about these issues during office visits. These posters are two-sided in English and Spanish and are suitable for display in both exam rooms and waiting areas.
