Preventive Medicine Residency and Fellowship (PMR/F)

Application Information

Application opens.

September 15, 2014

Postmark deadline for application documents (all supporting documents must arrive at the PMR/F office in one package).

Applications postmarked after the deadline will not be considered.

Incomplete applications will not be considered.

October to early November 2014

Interviews conducted via video-conferencing or in person in Atlanta, Georgia. In-person interviews are not required.

Candidates choosing to travel to Atlanta are responsible for their travel expenses.

Candidates must provide official transcripts, each in a sealed envelope, from the institution.

Mid-December 2014

Acceptance notification.

Mid-June 2015

Residency and fellowship orientation in Atlanta.

Late July 2015

Assignments start.

The application for the 2015 practicum has closed.


Supporting Documents

Mail all supporting documents in one package.


  • Place all supporting documents into one package
  • Each letter of recommendation must be in a sealed envelope
  • Transcript photocopies are permitted. However, official transcripts are required before interviewing

Letters of Recommendation

  • Three letters of recommendation are required from persons familiar with the quality of your work
  • Applicants in CDC-sponsored state or local assignments must submit letters of recommendation from their local and CDC supervisors
  • Provide the recommendation writers with the PMR/F Reference Letter Instructions and Template (3 pages; 28 KB)
    • Letters must be submitted in English
    • Letters must be specific to your application and dated within 3 months of the application
    • Each letter of recommendation must be submitted in a sealed envelope


  • Submit official transcripts for all degrees earned since high school (not required for current EIS officers)
  • If transcripts are not from a U.S. institution, provide other proof of degree completion (e.g., a diploma). Transcripts and proof of degree completion from non-U.S. institutions must be translated into English
  • High school transcripts are not required
  • Photocopies of transcripts are accepted. However, before interviewing, official transcripts sealed in an official envelope from each  institution are required

Documentation of Clinical Training

  • Submit a copy of certificates or letters from all sponsoring institutions verifying completion of accredited postgraduate clinical training (for example, ACGME)

Clinical License

  • Submit proof of a current, full, and unrestricted license to practice your qualifying clinical specialty in a U.S. or Canadian licensing jurisdiction
  • International medical school graduates (physicians) who apply for the fellowship are exempt

EIS Summary Report

  • Submit your most recent EIS Summary Report

  • Allow a few extra days for express mail delivery to reach the PMR/F office
  • Do not submit via fax or e-mail
  • Do not submit faxes or e-mails
