Heart Disease Death Rates for 1996 through 2000 for American Indian and Alaska Native Men Aged 35 Years and Older by County. The map shows the largest concentration of counties with the highest heart disease rates - meaning the top quintile – in South Dakota with other patches of top quintile counties found in Wisconsin, Michigan and Montana.
